Highlight on Transitional Care Unit
The Transitional Care Unit (TCU) is a 19-bed Medicare certified Hospital Based Skilled Nursing Facility. It is licensed by the state to provide skilled nursing care. The predominate patient population served is geriatrics. Occasional patient populations include adults. The TCU provides services to a wide variety of residents, including those residents that have complex medical problems. The most frequent diagnosis of patients is cerebral vascular accident, chronic obstructive pulmonary disease, pneumonia, medical back problems or heart failure. The TCU is able to do remote telemetry on patients requiring monitoring.
